Patients with chronic diseases require quality care, while facing the challenges of their conditions. Chronic Disease Solutions was established to assist physicians and their staffs to provide ongoing quality care to their patients, while reducing health care costs. Our experienced clinical team provides monthly services to Medicare patients according to the directives of Chronic Care Management Services (CCM). CCM requires physicians who participate in the CCM program to offer a minimum of 20 minutes per month of non-face-to-face care coordination. Ensuring receipt of all recommended preventive services.

Monitoring the patient’s condition (physical, mental, and social).

Providing patient transitional care, clinical coordination and medical guidance appropriate to medical needs.

In order to overcome the challenges of implementing CMS regulations and guidelines, we provide nurses and care coordinators who offer a high level of service on your behalf. Our experienced clinical staff takes on the burden of software development, staffing, and compliance with CCM. Furthermore, we provide administrative oversight to ensure that high standards are met and our mutual goals are accomplished. Our CCM software is designed to meet CMS regulations and provides clinical data for determining successful clinical pathways for our patients. Additionally, our software provides a variety of communication methods to your EHR and office staff via secure messaging. Communicating clinical findings creates a much needed bridge between patients with chronic diseases and your staff.
